Decima provides patient coordination, physician referral, protocol design consultation, and post-treatment tracking coordination services. Decima does not provide medical treatment, does not employ treating physicians, and does not administer any therapeutic agents. All medical treatment is provided by independent, licensed physicians and clinics in our network.

You agree to provide accurate and complete information in your patient intake. You acknowledge that the services are not a substitute for professional medical advice, diagnosis, or treatment. You are responsible for all decisions related to your medical care.
Initial physician review is provided at no cost. Treatment coordination fees and treatment costs are disclosed in writing before any payment is required. A $500 deposit is required to hold a treatment date and is applied toward total treatment cost. Refund policies are specified in your individual service agreement.
To the maximum extent permitted by law, Decima and its affiliates are not liable for any medical outcomes, adverse events, or complications arising from treatments coordinated through our services. Decima's liability is limited to the coordination fees paid to Decima, not including treatment costs paid directly to clinics or physicians.
These Terms are governed by the laws of the State of Florida, without regard to conflict of law provisions. Disputes shall be resolved in the courts of Miami-Dade County, Florida, or through binding arbitration as specified in your service agreement.
